Online case note entry allows
​our chaplains to document services rendered
​quickly & easily. 

     The chaplain service reporting page (CSR) has two separate forms that our chaplains use to document services rendered. The second of these forms, Case Notes, is at the bottom of the page. It consists of single text box and a submit button.
     Every time a chaplain provides contact hours for a client, a Case Note entry needs to be submitted, describing the contact. Case Notes are to be brief and simple. The intention of this system it to provide accountability without burdening chaplains with another lengthly task. Case notes are used to help keep the board of directors informed of the work that is being accomplished at the various agencies where we serve. Notes are reviewed at the monthly board meetings.
     Please observe the following rules when documenting case notes:
  • Never use a client's name in the case notes.  If needed, initials may suffice.
  • Document gains; decisions for Christ, baptism requests, etc.
  • Document needs; i.e. 'A.B. on unit 3 requested a Bible'
  • We encourage chaplains to set up access to the CSR page on their smartphones. It is possible to pull up the Case Notes and dictate them into your phone on the way home from a service, submitting the case notes without ever having to log into a computer.
